Policies

 

 

Payment for course is due in full on or before the day of the course.

 

Payment will be made by Cash, Business Check, Paypal or Credit Card.

 

Please make checks payable to Lifeline CPR.

 

Contract must be signed and returned by date stated on contract or this contract will be null and void.

 

There must be a minimum of five participants or a minimum payment of $300.00 for course to be held.

 

The actual number of participants may increase between the date that the contract is signed and the date of the course. The price adjustment will be made accordingly at the time of the course.

  

Failure to provide a 24 hour notice of cancellation will be subject to a $300.00 cancellation fee.

 

Instructor reserves the right to cancel the course due to inclement weather and reschedule the course for a future date.

 

Any alterations made to the contract will find it null and void.

 

Instructor will contact customer approximately 1 week prior to the scheduled course to confirm arrangements and answer any questions which the customer may have.

 

Instructor will provide the participant(s) with course certificate(s) verifying completion of the course.

 

The customer will provide a TV/DVD with remote and a location which the course may be taught.

 

Instructor will supply all other needed materials.

 

Returned checks due to NSF will result in a $30.00 fee.  

 

There is a $15 fee for each replacement card needed.

 

We are located out of Braidwood, IL. We service the towns within a 40 mile radius of Braidwood, IL.
